### Investment Property Loan Application

**Our application form is divided into two parts to ease the stress of the process.**

##### The first part requests the following:

- Your personal information
- The subject property information
- Your loan request amount and purpose
- Your total reserves available for this transaction
- Your entity information - to hold title to the property
- Date and sign

##### The second part collects:

- Your home owner's insurance (HOI) provider on the subject property
- Your preferred title company /  closing agent
- Legal declarations related to the loan
- Date and sign
